Program Self-Assessments

Each program is required to complete a self-assessment on a regular basis.  This process results in improvement plans with timelines for correction.  Data entry of  results are part of the Connecticut Birth to Three Data System – Service Providers Information Data Enrollment and Reporting (SPIDER) on the Accountability and Monitoring tab (A&M).  The A&M module uses the most Current Monitoring Measures and each item has an information button that describes how to assess each measure in greater detail.

Programs are placed into one of three groups based on their monitoring history and self-assessments are completed every three years.  The first group has been notified and their self-assessments are due no later than September 9, 2013.
