Family Survey Clarification

Each March we give batches of pre-printed family surveys to programs to pass on to specific families. The families who get the surveys are those who have had more than six months of visits on Feb 1st each year.
If you have a family who you think should have received a survey and didn’t, first please check that their first visit was before 8/1/13. If it was, please contact Anna.Hollister@CT.gov. If it wasn’t, please do not give the family a blank survey as they need a number to complete the survey online. In addition, regardless of how the survey is completed, their results will not be counted if the number doesn’t match one on the list that we sent to each program.
