The Forum, hosted by Connecticut Children’s Center for Care Coordination, convenes professionals from all sectors who work with children and families to learn from experts and exchange ideas. Our goal is to organize coordinators from all areas; healthcare, community-based, education, behavioral health, and more- in an effort to provide an effective and comprehensive care coordination system.
