E-signatures are Okay to release child record to SSA

The Social Security Administration’s (SSA’s) secure online application and electronic medical record system use electronic signatures to indicate Parent/Guardian consent to release personal information about their child.  If you are wondering if this electronic signature is acceptable to release the child’s Birth to Three records, the answer is YES. Medical Examiners rely on Birth to Three evaluations as part of their determination of eligibility for cash benefits. Family Survey Clarification

Each March we give batches of pre-printed family surveys to programs to pass on to specific families.  The families who get the surveys are those who have had more than six months of visits on Feb 1st each year. If you have a family who you think should have received a survey and didn’t, first please check that their first visit was before 8/1/13.  If it was, please contact Anna.Hollister@CT.gov.   If it wasn’t, please do not give the family a blank survey as they need a number to complete the survey online.
